Person Centered Planning

Person centered planning is a best practice in determining transition needs, strengths and interests. The goal of a person centered planning session is to bring together friends, family and professionals that can speak to and support developing a vision for a youth.

How to learn more about facilitating a person centered planning session?

1. Check out a webinar from the Federation for Children with Special Needs on Creating a Post-Secondary Vision and the person centered planning process.

2. Learn more about person centered planning and some tools from the Arc of Massachusetts and their Support Brokers team.
