What is Transition-to-Adulthood?

Looking for a Roadmap or Timeline? 

The transition to adulthood is the period between ages 14 and 26 when youth and young adults with disabilities prepare for life after high school. Along the way, families make important decisions about education, employment, adult services, independent living, healthcare, and community participation.
Join specialists from four organizations for a practical, age-by-age guide to transition planning. Creating an interactive timeline, presenters will highlight the key milestones, resources, and action steps for each stage of the journey, so you’ll know what to do, when to do it, and where to find support.
