Additional Support Services

At FCSN, our mission has always been to ensure that families have access to the information, support, and resources they need. Through our grant-funded programs, we continue to provide free information and referral services, trainings, and guidance to families and professionals across Massachusetts.

As the needs of families and systems continue to grow, we have also developed a range of additional support services designed to provide more in-depth, individualized, or specialized assistance. These services—offered on a fee-for-service basis—allow us to expand our impact, dedicate additional time and expertise, and respond to complex or intensive support needs.

We are committed to ensuring that cost is not a barrier. Scholarship and reduced-fee options are available, and we encourage you to reach out to learn more.

Explore the options below to find the level of support that best meets your needs.

Transition Clinic

Transition Clinics are an opportunity for families to receive support over zoom from an experienced member of the Individual Assistance/Autism Housing Pathways team at FCSN.

Transition Clinics are appropriate for students on a life skills/community supported path, students on a path to higher education and independent living and everything between.

Ages 14–18: Focused on evaluating the needs of the student, planning, and building skills to support successful adult outcomes.

Ages 18–22: Focused on strengthening transition services, refining post-secondary goals, applying for government benefits and preparing for life after high school.

Over age 22: Focused on navigating adult services, community support, employment options, and long-term planning.
